How to register
Register now to enjoy one month's free membership with Fusion Leisure Centres and become a Concordia patient. You can register online now or just pop into reception and we will talk you through the process.

Once registered, we can request your medical records from your previous practice via Southwark Primary Care Trust.

Please remember to notify us if you change your name, address or telephone number. You can notify us online, by fax or post, or just pop in and inform reception.

Your responsibilities as a patient
Our nurses and doctors are here to help, and we aim to provide you with the best possible care. In order for us to do this we expect you to be considerate, courteous and not to make unreasonable demands. You can leave a message or telephone us when a face-to-face contact is unnecessary. This will help us to function more effectively and allows us to be more available.
